NOTES ON
SUBMITTING EXAMINATION APPLICATIONS
Please read these notes very carefully: Implementation
is with immediate effect
- Documents required to accompany the application form
must be certified.
- When completing the application form please print
clearly. Use the full name as it is stated on your ID document.
- If the handwriting on the application form is illegible
your documents will be returned to you.
- Incomplete forms and/or application forms that do not
include all the required documents will be returned to you.
- If your documents are returned to you for any reason
the re-submitted forms must still reach this office before the closing date.
It is therefore very important that you check the documents before
posting them to the office.
- The full examination fee must be sent with the
application form and the documents.
- Application forms and documents for entry to the Board
examination may only be posted to this office.
- Please ensure that the correct postage is affixed to
your mailing envelope. Insufficient postage incurs a cost to this office
when mail is collected. The deficient amount of postage is doubled. The
mail will not be accepted and it will be returned to you.
- Faxed documents will be discarded.
- The post office date stamp on the envelope must not be
later than the closing date of the examination.
- Closing date for acceptance of applications for
Technologist examinations in March and September and Technicians in October is
31 May each year.
Postal address:
- Please advise this office immediately in writing of any
change of postal address.
- The postal address stated on the application form is the
address that will be used for all the correspondence sent to you regarding
your examination. It is therefore imperative that you receive this mail.
- This office takes no responsibility for mail that you do
not receive, should you state an address at which you are not living or do not
have access to the post.
- The onus is on you to ensure that the correspondence
reaches you.
Unable to write the examination:
In the event that you are unable to write the examination
the following must be carefully understood.
- You may not transfer the examination to a later date
- Your examination fee will not be refunded.
- You are required to re-apply to write the examination at
a later date and re-submit all the certified documents together with the
examination application form and fee to this office.
- The application documents are to reach this office
before the closing date for entry to the examination. The post mark on the
envelope must be no later than the closing date.
